Follow Each Step Carefully Before Making an Appointment

  • Applications are accepted by appointment only at the Township Clerk’s office Monday through Wednesday between 9 am and 2 pm.  Appointments are made online only. 
  • If you are more than 15 minutes late for your appointment, or you do not have all of the correct documents, you will be required to make an appointment for a later date.
  • Do you need to renew your passport?  In most cases, adult passport renewals must be submitted by mail and cannot be completed/processed at a Passport Acceptance Agency.  Click here to find out if you are eligible for renewal by mail.

  • All applicants must be present, including minors applying with their guardians. No more than four applications will be processed during one appointment slot. If you need to process more than four applications, please make more than one appointment.

What You Need For A Typical Passport

  • Completed application
    • You may complete the application online at www.travel.state.gov and print the completed application or obtain one from a Passport Acceptance Agency before your appointment.  If you print the form, it must be single-sided.  The U.S. Department of State does not accept double-sided forms.
  • Valid Driver’s License or State ID
  • Certified Birth Certificate with raised seal or Naturalization Certificate
  • If there has been a name change, bring original name change documents (court documents signed by a Judge, or a document with a raised seal, such as a certified marriage license)
  • Passport Picture
  • Two separate payments
    • One payment to the US Dept of State (Check or Money Order only)
    • One payment to Cascade Charter Township (Cash, Check, or Money Order)
Passport Photo Requirements
  • Color photo, taken in the last 6 months
  • Must be a clear image of your face without any filters
  • No selfies
  • No eyeglasses
  • Plain white or off-white background without shadows, texture, or lines
